Insurance errors
One visit, two bills: the professional and the facility claim
Understanding which bill is which stops you from paying twice or disputing the wrong one.
Care delivered in a hospital, surgery center, or emergency department usually generates two separate claims, and each one produces its own bill.
| Professional claim | Facility claim | |
|---|---|---|
| Who bills it | The clinician or practice | The hospital or facility |
| Claim form | CMS-1500 | UB-04 |
| Covers | The physician's work and interpretation | The building, nursing, equipment, drugs, supplies |
| Can be missing | Yes | Yes, and more often |
Why this causes confusion
- You can pay the doctor bill and still have a hospital bill arrive months later.
- The two amounts do not relate in any arithmetic way.
- Insurance processes them independently, with separate claim numbers and timelines.
- A hospital can have the facility claim missing while the professional claim paid cleanly.
What to do
- Ask your insurer about both claims for the date of service, and get a reference number for each answer.
- Ask the facility for the facility claim number and the date it was submitted.
- Compare each bill to the matching EOB rather than to the other bill.
- Request the itemized bill for both, since they carry different codes.
In our own case one side of an emergency visit was not received by the insurer for roughly two months, and we only found out by asking about that side specifically. Nobody had mentioned it.
